BBEG Lucius Bochs, Criminal Kingpin of Sheol (City of Woe, d&d5e)

Stop me if you've heard this one. 

Lucius Bochs, uncrowned King of Nessus, the lowest of 9  layers of the City of Sheol,  built inside an active volcano that protects the city from a frozen devastated world outside. A mountain of a man in a white suit, a barbarian cloaked in civility. The Bochsers are his enforcers, and he controls the lion's share of crime in  Sheol.

A very dangerous BBEG for a Noir Urban setting like the one outlined in DC Bradshaw's Hardboiled Adventures.
